I voted Cleveland overall, but for poker alone it's Columbus for me (unless you play higher stakes PLO, which I don't, in which case Cincinnati bridges the gap.)
Cleveland's rake on poker tournaments is incredibly high, and for NLHE their 1/3 killed the 2/5 action a lot once it switched over from 1/2.
More of the money raked from pots in Cbus goes to the players ($5 rake + $2 promo max instead of $6 rake + $1 promo max) and their poker room manager is active on 2p2 and is easily accessible with any thoughts or concerns.
I would have voted for Cincy overall, but now it's not even an option for me. I don't have a lot of bad to say about the poker room, but I can't call it my overall favorite anymore.
Toledo just doesn't have enough traffic. It's on the way when I'm going home to see my family, and I have no problem stopping there every time.
All 4 are actually very nice casinos, and I still love the fact they're non-smoking. I really hope it stays that way.
