So who is going to order the Spinal Tap themed set where all the spots go to 11? It'd be nice to have these provided in the design tool, like the gambling addiction 800 number on casino advertising.

It looks like the level 8 4DSA18-esque spot pictured has 8 different color spots (each of the 8 spots is different), whereas the level 4 4DSA18 only had 2 colors (four of one color, 4 of another).

4DSA18 is still Level 4 (look on the regular spots page)
4DSQA18 is level 8.
When multiple color spots are used, the chip has to be made in more stages. Same applies to all spot patterns.
Remember that most of the complex patterns in the PDF are just multiple color variations of regular patterns which people would not ordinarily use.

There have been NO changes to any spot levels.

There is no 418 punch. Only exists in 118, 218 & 318. You cannot punch 4 times, it would break from being heated and cooled so much. That is why it doesn't exist. Even if you could it would be level 16 as that is like making 4 chips.

418 punches used to exist but broke all the time. I will try and explain simply why.
Obviously 1/8 wide pins are very small and not very strong. There comes a point when the material you are punching is almost as strong as the punch, and unless all pins struck the chip surface at precisely the same millisecond one can easily bend or break. It is very hard to make 4 pins come into contact at exactly the same time, even allowing the chip flexes a fraction.
Take a 4 legged bar stool and cut a little off one leg. It will wobble. Impossible for all 4 legs to touch the floor (if it is flat :) ) at the same time. 3 legged stool is very stable though. Cut a bit off a leg and it still balances.

Don't forget also that in addition to the complexity of assembling the example discussed with 8 different colors (or indeed anything more than 4 colors repeated) you also have to make clay, slugs and then spots in all those different colors.
The spot levels are all calculated based on a combination of time taken to make those components, assemble them, and differing wastage, based on physically observing and timing those processes. That is why some that look simpler visually may be in higher levels than some that look more complex - there could actually be less stages involved. Rather than say we can't do something because it is more trouble than it is worth we took the view that it was better to just try and accurately price everything accordingly. In reality, profit on a Level 12 chip will be no more than on a plain chip. And as mentioned in caveats on other occasions, we would not always agree to do a high quantity of a very complex pattern regardless of price as it could be too much strain on employees (seriously), unless the customer is happy to wait 6 months.

And of course in contrast to the 1/8 spots, we have single punches to do 614s and 814s, hence they appear more modestly priced in relation to other patterns.
